Make a Referral for Evaluation
Thank you for considering MAPS for your child’s diagnostic evaluation. To help us provide the most thorough, effective assessment, please fill out the form below and include as much relevant information as possible.
We ask that you attach any prior evaluations or records, such as:
- Speech or occupational therapy assessments
- Psychological or neurological testing
- IEP/IFSP reports
- Medical history or developmental milestone records
If a community provider is making a referral, please attach an ROI so we can connect with you. Evaluations will take place in Bloomington at our office location, unless scheduled otherwise.
